These questions bear on the person we are planning for.  Please try to finish all applicable questions and bring in all the requested documents.  Do not worry if cannot complete all of the questions before the first meeting or you are unable to bring all requested documents with you.  Please give your best estimate of what you think items are worth; don’t worry if you don’t know exactly.  Please bring with you copies/originals of all existing planning documents, such as wills, trusts, and powers of attorney. 

At our meeting we will review the information on this questionnaire, discuss your goals and issues, and discuss fees for our services and document preparation beyond our first meeting.  You are not under any obligation to retain us.  Should you decide to retain us you will receive a fee schedule detailing the financial arrangements we agree on.
